Applying Geriatric Assessment Across Settings and Complex Cases

Comprehensive geriatric assessment is most effective when adapted to the setting and integrated into team-based care for complex patients. This applied course teaches that adaptation with case-based modules. By the end of this course you will be able to adapt CGA to hospital, primary care, long-term care, and home settings, integrate CGA into multimorbidity care, coordinate across the interdisciplinary team, and use CGA to support transitions of care. What you will learn: - Adapting CGA structure to hospital admissions, including risk stratification - Adapting CGA to outpatient primary care and chronic disease management - Conducting CGA in long-term care and home settings with environmental focus - Using CGA to support multimorbidity care including disease prioritization - Integrating CGA findings into interdisciplinary team rounds and family conferences - Using CGA at transitions of care to reduce adverse outcomes - Building CGA-informed quality improvement processes in geriatric services The course is structured around six applied modules. Each produces a written artifact: a setting-adapted CGA plan, a multimorbidity prioritization framework, a team integration plan, a transition support plan, a complex case reasoning template, and a quality improvement sketch. This course is written for clinicians and team members deepening geriatric practice. It does not replace formal geriatric training or supervised practice. By the end you will have structured frameworks for applying CGA across settings and complex cases.
