About this course
Delivering multiples is not simply delivering a singleton birth twice. The clinical considerations are different, the monitoring requirements are more complex, the role of fetal positioning is more critical, and the probability of cesarean birth is substantially higher. Expecting parents of multiples often receive fragmented information and benefit enormously from a comprehensive, honest picture of what their delivery options actually are and how those options are determined.
By the end of this course you will be able to describe the clinical factors that influence delivery mode for multiples, explain how fetal position determines vaginal birth eligibility for twins, describe what a vaginal twin birth looks like from a clinical perspective, and articulate what to expect during a planned cesarean birth of multiples.
What you will learn:
- The clinical categories of twin pregnancy — dichorionic-diamniotic, monochorionic-diamniotic, and monochorionic-monoamniotic — and how each category affects delivery timing and mode
- How the position of Twin A and Twin B determines eligibility for vaginal birth, and how position can change in the final weeks of pregnancy
- What a vaginal twin delivery looks like: the monitoring protocols, the time window between deliveries, the management of Twin B's position after Twin A is born
- How an internal cephalic version or external version is used to manage an unfavorable position of Twin B after vaginal delivery of Twin A
- The clinical indications that make cesarean delivery the recommended or required approach for multiples
- What a planned cesarean birth of twins looks like in practice: anesthesia, delivery sequence, cord clamping timing, and the immediate postpartum period
- How triplet and higher-order multiple births differ from twin births in terms of delivery protocols
- How to have an informed conversation with your obstetric team about delivery mode, and what questions to ask at each prenatal visit as delivery approaches
This course is organized as four readings covering the clinical landscape of multiples delivery. The first reading explains the classification of twin pregnancies and their implications. The second covers vaginal twin birth in detail. The third covers cesarean birth for multiples. The fourth addresses triplets and the final prenatal preparation period. Each reading includes a reflection prompt, and a summary reference chart compiles the key clinical decision points.
This course is designed for expecting parents of multiples who want a thorough, evidence-grounded understanding of their delivery options. No prior background is required. This course is informational and does not substitute for detailed discussions with your maternal-fetal medicine specialist and obstetric care team.
