Understanding Severe Postpartum Mood Disorders: Postpartum Psychosis and Postpartum Bipolar
A factual, non-sensational introduction to the rarest and most severe postpartum mental health conditions — what they are, how they present, and why rapid recognition and response matter.
About this course
Postpartum psychosis and postpartum bipolar disorder are rare, but they are psychiatric emergencies that require immediate professional intervention. Families who are aware of these conditions — what the warning signs look like, how quickly they can develop, and what the correct response is — are better equipped to recognize them and act quickly. This knowledge can be lifesaving.
By the end of this course you will be able to describe the clinical presentation of postpartum psychosis and postpartum-onset bipolar disorder, identify the symptoms that constitute a psychiatric emergency, and explain the appropriate steps for seeking immediate evaluation and care.
What you will learn:
- The clinical definition of postpartum psychosis: its onset timeline, the symptom cluster that distinguishes it from other postpartum mood disorders, and its typical course without and with treatment
- The specific symptoms that constitute an emergency presentation: confusion and disorientation, rapid mood cycling, auditory or visual hallucinations, delusional thinking, and behavioral disorganization
- How postpartum psychosis differs from postpartum depression and from other causes of acute mental status changes in the postpartum period
- The concept of postpartum bipolar disorder, how it presents, and why its recognition requires a different treatment approach than unipolar postpartum depression
- The risk factors most strongly associated with postpartum psychosis, including personal or family history of bipolar disorder, a previous postpartum psychosis episode, and abrupt sleep deprivation
- Why postpartum psychosis is considered a time-sensitive emergency and what outcomes are associated with rapid versus delayed treatment
- The appropriate emergency response: when to call emergency services, what to tell them, and what inpatient psychiatric evaluation and treatment involves at a basic level
- How these conditions are discussed in a way that is honest about their seriousness while remaining respectful of the people who experience them and their families
This course is organized into three sections: the clinical landscape of severe postpartum mood disorders, symptom recognition and emergency thresholds, and response and treatment overview. Each section uses clear, direct language without sensationalism. Case vignettes are drawn from composite experiences and are written with care and respect. Reflection prompts help you consolidate the material rather than dramatize it.
This course is written for expectant and new parents, their partners, family members, doulas, and childbirth educators who want to be prepared to recognize and respond to these rare but serious conditions. No prior background in psychiatry is required. This material is educational and does not substitute for clinical evaluation or treatment. If you observe symptoms consistent with postpartum psychosis in someone who has recently given birth, treat it as a medical emergency and contact emergency services or take the person to an emergency room immediately.
